Our Disability Management Team continues to take a proactive approach to managing symptoms of COVID-19 and suspected exposure.  As a result, we have updated the Employee Daily Health Check to align with current public health guidance:

Please read all the questions carefully every time. The following symptoms have been deleted from the Employee Daily Health Check:

Question #1

  • Sore throat or difficulty swallowing
  • Pink Eye
  • Runny or Stuffy/congested nose
  • Digestive issues (nausea/vomiting, diarrhea, stomach pain)

Question #2

  • Headache

What this means for you:

The symptoms associated most frequently with COVID-19 have changed over time.    Complete your Daily Health Check, even if you know you are not reporting to work.  Employees receiving a red X will be required to contact DMS at (519) 258-6000, ext. 22322 or email

We continue to encourage employees who are sick to stay home.  If you are sick for a reason not outlined in the daily health check, follow-up with DMS is not necessary.  If your symptoms change at any time during your illness, take the online health screening at or contact Telehealth Ontario at 1-866-797-0000 and follow their direction.  Contact DMS if your new symptoms may be related to COVID-19.

Additionally, the following questions have been amended (IN RED):

Question #5:

Within the past 10 days have you been a “close contact” of someone who currently has COVID-19?   If you are fully vaccinated and/or if public health has advised you that you do not need to self- isolate, select “No”.

Question #6:

In the past 10 days have you received a COVID Alert exposure notification on your cell phone?  If you are fully vaccinated and/or already gone for a test and got a negative result, select “No”.

Question #8:

Is anyone you live with currently experiencing any new COVID-19 symptoms and/or waiting for test results after experiencing symptoms?   If you are fully vaccinated, select “No”.
